Student data is printed on an OSPI form. Nothing additional can be printed on the form that is not specifically authorized under WAC 392-415. A school district can, however, attach other documents to the standardized transcript.
The WESPaC transcript report was developed using a programming tool called Viper. This allows the transcript form to be created at the same time as the student’s data. The transcript report must be printed on a laser printer to utilize this type of forms overlay. It is not necessary to load the laser printer with a special form.
The standardized transcript form is comprised of three main sections:
The Header contains:
Student Information
School of Record Information
Administrative Information
The Body contains:
The Footer contains:
Information about OSPI
Signature
Page Numbers
The sample standardized transcript below is for a student with Washington Grad Year = 2008.
